But she can also go outside so she will find a way to get water

I think most people scoop some of the mess out every other day and clean the whole thing every week? Depends on the smell and size of the cat

They learn it from their mother. That's why it's important that kittens spend the first 1 to 2 months after birth with her. But I think it's also their instinct. They look for something that's diggable like sand or soil to shit/piss in.
These fountain bowls are good for cats that like moving water. My friend has one because their kitty doesn't drink from a regular bowl.
