Notes from Cathriona's White therapy sessions between October 2012 and July 2013 have been obtained by DailyMail.com
The notes detail her relationship with Jim Carrey at that time, and include her claim that the actor had given her STDs after lying about a Herpes outbreak

After learning she had contracted STDs, White became unable to eat or sleep according to the therapist's notes and had anxiety-induced vomiting
The session notes also reveal that White 'tried to commit suicide on with a bottle of painkillers' she claimed were from Carrey on January 8, 2013

A source close to the case said that any pills White took would have been her own from a root canal procedure she had undergone a few weeks prior
A week after that attempt she married estranged husband Mark Burton in Vegas, but on January 30 told her therapist she was upset Carrey would not marry her

White took her own life on September 25, 2015 and a year later her mother and estranged husband filed a wrongful death lawsuit against Carrey 

His lawyer Marty Singer previously called the lawsuit a shameless shakedown and says his client is blameless

DailyMail.com revealed last October that the STD test submitted as evidence in the wrongful death suit filed against Jim Carrey by Sweetman showed positive results for multiple diseases.

The Quest Diagnostics test belonging to 'Jose Lopez' and filed under seal has been seen by DailyMail.com and shows that 'Lopez' tested positive for Herpes I and II, Chalmidya and Hepatitis on January 28, 2013, with the results being reported three days later according to the test.

The date of birth for Lopez is listed as January 17, 1962, which is also Carrey's date of birth.
